In this vital role, you will provide compassionate patient care Primarily LTC. Your expertise will help ensure the delivery of high-quality nursing services, promote patient safety, and support overall well-being. This position offers an exciting opportunity to work with diverse patient populations, including geriatric and developmentally disabled individuals, while utilizing your clinical skills in a fast-paced environment. Duties Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient needs. Administer medications and injections accurately, including IV infusions and other specialized treatments as needed. Manage complex cases involving dementia care, Alzheimer's disease, memory care, and behavioral health issues. Document all patient interactions thoroughly using electronic health record (EHR) PCC Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care strategies to optimize patient outcomes. Provide emergency response support including airway management, acute pain management. Experience Proven experience working in diverse healthcare environments such as Long Term care centers is highly desirable. Hands-on experience with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for medical documentation and records management. Strong background in managing complex medical conditions including infectious diseases, chronic pain management, hospice care, and geriatrics. Experience working with vulnerable populations such as Geriatric, and those requiring specialized dementia or Alzheimer's care.
