Responsibilities Conduct comprehensive wound assessments, including measuring, documenting, and evaluating wound progress using EMR/EHR systems. Develop and implement individualized wound care plans aligned with best practices and patient needs. Administer treatments such as dressing changes, IV infusions related to wound management, and medication administration with precision and adherence to protocols. Monitor patients' vital signs, pain levels, and overall condition during each shift to ensure optimal healing outcomes. Collaborate closely with physicians, case managers, and interdisciplinary teams to coordinate care plans and discharge planning. Educate patients and their families on wound care techniques, infection prevention, and self-care strategies to promote healing at home. Maintain meticulous medical documentation in compliance with HIPAA regulations and facility standards while managing electronic health records (EHR) efficiently. Experience Valid LPN or RN licensure with current state certification required; additional certifications in wound care or IV therapy preferred. Critical care experience such as ICU or emergency medicine enhances your ability to handle complex cases effectively. Prior experience working in long-term care facilities, skilled nursing facilities, assisted living communities, or hospital settings is highly desirable. Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth ensures seamless documentation workflows. Demonstrated knowledge of infection control practices, sterile techniques, and aseptic procedures essential for safe wound management. Experience working with diverse populations including geriatric patients, individuals with developmental disabilities, dementia or Alzheimer's disease enhances patient-centered care delivery. Strong clinical assessment skills coupled with expertise in pain management, dressing selection, catheterization, and tube feeding are advantageous.
