Duties Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient needs. Assist with medical procedures such as tube feeding, wound suturing, phlebotomy, and sterile processing techniques. Manage electronic health records (EHR/EHR systems) by documenting medical histories, treatment plans, and progress notes in compliance with HIPAA regulations. Provide specialized care for patients with dementia, Alzheimer's disease, memory impairments, and behavioral health conditions. Support acute pain management strategies and participate in discharge planning for post-acute care patients. Collaborate effectively with physicians, case managers, caregivers, and interdisciplinary teams to ensure optimal patient outcomes across settings like ICU, emergency room, or skilled nursing facilities. Conduct triage assessments in urgent or emergency situations to prioritize patient needs based on severity levels such as Level I Trauma Center or Level II Trauma Center experiences. Educate patients and their families on health coaching topics including medication administration, nutrition, mobility assistance (e.g., Hoyer lifts), and infection control practices. Ensure compliance with long-term care regulations and safety standards during all patient interactions. Experience Valid Licensed Practical Nurse (LPN) license with current state certification required. Demonstrated experience working with diverse populations such as geriatrics, pediatrics—including neonatal ventilator care—and individuals with developmental disabilities or disabilities requiring specialized attention. Prior experience in medical-surgical units, emergency medicine (ER), ICU or critical care environments preferred. Familiarity with EMR/EHR systems such as Cerner or Athenahealth for accurate documentation and record management. Knowledge of ICD coding (ICD-9/ICD-10) for diagnostic evaluation and medical documentation accuracy. Experience providing hospice or palliative care services is a plus. Skilled in performing procedures like venipuncture, catheterization, airway management (ventilators), first aid techniques, and infection control protocols. Strong understanding of anatomy and physiology to support effective patient assessment and treatment planning. Ability to work effectively in fast-paced settings such as hospital medicine units or outpatient clinics while maintaining clinical confidentiality policies.
