Full benefits package + gas mileage reimbursement About the Role We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team in Edmond & North West OKC. The ideal candidate will have at least 3 years of home health nursing experience and a passion for providing high-quality, patient-centered care in the home setting. This role offers flexibility, autonomy, and the opportunity to build meaningful relationships with patients and families.

Qualifications:

Current Licensed Practical Nurse (LPN) license in the state of Oklahoma. Minimum of 3 years of nursing experience (home health experience required). Strong clinical assessment and documentation skills. Reliable transportation and a valid driver's license. Compassionate, dependable, and committed to providing exceptional patient care.

Responsibilities:

Provide direct nursing care to patients in their homes under the supervision of an RN and in accordance with the plan of care. Conduct patient assessments, monitor vital signs, and accurately document progress. Administer medications and treatments as prescribed. Educate patients and families on care plans, medications, and health maintenance. Communicate changes in patient condition promptly to the supervising RN and physician. Ensure compliance with agency policies, procedures, and state/federal regulations. Travel to patient homes within NW OKC & Edmond (mileage reimbursement provided).
