This position provides direct nursing care to adults receiving substance use detoxification and residential treatment. The LPN works closely with medical providers, nursing staff, therapists, behavioral health technicians, and other members of the interdisciplinary treatment team to support safe withdrawal management, medication administration, ongoing medical monitoring, and recovery-focused care. We are looking for a nurse who is clinically attentive, comfortable working in behavioral health, and able to recognize changes in a patient's condition and respond appropriately within the LPN scope of practice. Responsibilities Provide direct nursing care in a substance use detoxification and residential treatment setting. Administer medications and treatments according to provider orders and facility protocols. Monitor patients for signs and symptoms of alcohol, opioid, benzodiazepine, and other substance withdrawal. Complete withdrawal-monitoring assessments such as CIWA-Ar and COWS as assigned and within scope of practice. Obtain and monitor vital signs, oxygen saturation, weight, and other clinical measurements. Observe for changes in physical condition, mental status, level of consciousness, withdrawal severity, medication response, and overall medical stability. Promptly report significant changes in patient condition to the RN, Nurse Manager, or medical provider. Administer medications used for withdrawal management and substance use treatment as ordered. Support Medication-Assisted Treatment (MAT) and other medications for substance use disorders according to provider orders and established protocols. Monitor patients for medication effectiveness, side effects, adverse reactions, oversedation, and other safety concerns. Maintain accurate medication administration records and electronic health record documentation. Assist with medication reconciliation, inventory, medication counts, storage, and medication-room procedures. Provide patient education regarding medications, withdrawal symptoms, health concerns, hydration, nutrition, and recovery-related health needs. Recognize potential medical emergencies and initiate appropriate emergency response procedures within scope of practice. Communicate with emergency medical services and other healthcare providers when a patient requires a higher level of medical care. Participate in shift-to-shift nursing communication and interdisciplinary treatment-team coordination. Maintain patient confidentiality and professional boundaries. Follow infection-control, medication-safety, patient-safety, and facility policies. Complete incident reports and other required documentation when medication errors, adverse events, or significant medical concerns occur. Provide respectful, trauma-informed, recovery-oriented care to individuals with substance use and co-occurring behavioral health conditions. Qualifications Current and unrestricted Licensed Practical Nurse (LPN) license in Oklahoma or authorization to practice in Oklahoma. Current CPR/BLS certification. Strong medication-administration and dosage-calculation skills. Ability to recognize changes in patient condition and appropriately escalate medical concerns. Strong documentation and communication skills. Ability to work effectively as part of an interdisciplinary treatment team. Ability to maintain confidentiality, professional boundaries, and a calm approach in a behavioral health environment. Preferred Experience Experience in one or more of the following areas is preferred: Substance use disorder treatment Medical detoxification Behavioral health or psychiatric nursing Residential treatment Inpatient nursing Withdrawal management CIWA-Ar and COWS assessments Medication-Assisted Treatment (MAT) Co-occurring mental health and substance use disorders Previous addiction-treatment experience is preferred but is not required for a qualified nurse who is interested in learning this specialty.
