1 Description American Traveler is hiring an LPN/LVN for a Home Health & Hospice setting, requiring 2+ years of Home Health experience and hospice experience. Details Home Health and Hospice work setting focused on routine visits Patient ratio of 1:30 Epic charting system experience required Day shift schedule, 5x8 hours, 8:00 AM-4:30 PM Possible evening/night on-call responsibilities May be required to float within the health system as operationally necessary Requirements Washington LPN/LVN license required if applicable Minimum 2 years of experience in Home Health required Hospice experience required BLS certification required Epic charting experience required Two professional references from the past 2 years required, at least one from a supervisor Proof of Washington state license application required if license not yet in hand Travel healthcare experience required Candidates with more than one DUI are not eligible for consideration Additional Information Provide routine home health visits including hospice care Previous PeaceHealth employees must be separated from the health system for at least 1 year before returning as a traveler First-time travelers will not be considered for this position Local candidates within 60 miles of the facility are not accepted RTO must be disclosed and approved prior to contract signing, with no additional RTO approved afterward Start date may be delayed up to 8 weeks if Washington license is still in process Proof of full vaccination status must be disclosed
