Qualifications:

A valid certification or degree as a licensed practical nurse (LPN) or licensed social worker (LSW). An active and unrestricted license in the relevant state for this position. Three-plus years of experience in hospital or post-acute care settings. Proficiency in technology, including electronic medical systems and Microsoft Office. The position requires a passion for improving patient outcomes and working in a collaborative team environment. Authorized to work in the United States.

Scheduling:

Enjoy flexible scheduling in a fast-paced environment with a Monday through Friday workweek from 7:00 AM to 4:00 PM (including a lunch break). No nights, no weekends, and paid holidays.

Key Responsibilities:

Partner with other Sound nurses, site leaders, and the hospitalist team to improve the quality and efficiency of hospitalized patient care. Report to nursing leadership. Improve patient outcomes by promoting health, preventing readmission, and addressing barriers to healthcare access before discharge. Work to improve the patient experience and optimize resource utilization. No direct patient care. Interact directly with patients at the bedside through patient satisfaction and discharge preparedness conversations. Living and Working in
