Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have a strong background in patient care, with experience across various medical settings including long-term care, assisted living, hospitals, and outpatient clinics. This role involves providing high-quality nursing services, supporting patient recovery and well-being, and ensuring compliance with healthcare standards. The LPN will work closely with multidisciplinary teams to deliver comprehensive care tailored to individual patient needs, including those with complex conditions such as dementia, Alzheimer's, or developmental disabilities. Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth is preferred. Duties Perform comprehensive patient assessments including vital signs, physical examinations, and health history documentation. Administer medications and injections accurately following physician orders while adhering to HIPAA regulations and sterile techniques. Manage IV infusions, catheter care, tube feeding, and ventilator support as required. Assist with diagnostic evaluations such as laboratory specimen collection, processing, and medical imaging procedures. Provide wound care including suturing and dressing changes; support post-operative recovery and discharge planning. Monitor patients' physiological status using anatomy and physiology knowledge; recognize signs of acute pain or deterioration requiring urgent intervention. Support patients with dementia or memory care needs through specialized approaches; assist with activities of daily living in senior care settings. Document all patient interactions thoroughly in EMR/EHR systems like eClinicalWorks or Cerner; ensure compliance with medical records standards. Collaborate with physicians and other healthcare professionals in case management, triage, and discharge planning processes. Educate patients and families on health coaching topics such as medication management, nutrition, or chronic disease management. Maintain infection control protocols and aseptic techniques to prevent healthcare-associated infections. Requirements Valid state licensure as a Licensed Practical Nurse (LPN). Proven experience in diverse clinical environments such as hospitals (ICU, ER, trauma centers), nursing homes, hospice care, or outpatient clinics. Familiarity with

ICD-9/ICD-10

coding systems; experience with medical documentation standards is highly desirable. Proficiency in EMR/EHR systems including Epic, Cerner, Athenahealth; strong medical terminology knowledge required. Experience administering injections, IV insertion/maintenance, phlebotomy, catheterization, and other invasive procedures. Knowledge of infection control practices and sterile processing techniques. Ability to perform physical assessments accurately; understanding of physiology and anatomy essential. Excellent communication skills for working with patients of all ages—including pediatrics (neonatal ventilator care) to geriatrics—and their families. Ability to work independently while collaborating effectively within multidisciplinary teams. Must demonstrate critical thinking skills in triage situations; capable of managing acute pain or emergency scenarios such as airway management or spinal tap procedures.
