Schedule:

Five 8-hour shifts per week Rotating shifts—flexibility required

Key Responsibilities:

Deliver compassionate home health care to patients in accordance with individualized care plans Collaborate with multidisciplinary teams to optimize patient outcomes Complete timely and accurate documentation for care episodes and patient progress Educate patients and families on health maintenance, safety, and disease prevention Adhere to best practices, all relevant state regulations, and patient care standards

Qualifications:

Successful completion of a Licensed Practical Nursing Program approved by the State Board of Nursing Minimum of 2 years of experience as an LPN, preferably in home health Valid and unrestricted Pennsylvania LPN license Current Basic Life Support (BLS) certification Compliance with PA Act 31, PA Act 33, PA Act 34, and PA Act 73 requirements
