Responsibilities include:

Administer medications and treatments as prescribed while monitoring residents for therapeutic effectiveness and adverse reactions. Obtain and accurately record vital signs and other clinical observations. Assess, monitor, and document resident conditions, reporting changes promptly to Registered Nurses and physicians. Maintain complete, accurate, and timely clinical documentation in accordance with facility policies and regulatory requirements. Assist in developing and implementing individualized resident care plans. Communicate effectively with residents, families, physicians, and interdisciplinary team members. Provide compassionate, resident-centered nursing care while promoting dignity, independence, and quality of life. Supervise and collaborate with Certified Nursing Assistants to ensure exceptional resident care. Ensure compliance with facility policies, state and federal regulations, and infection prevention standards. Respond appropriately to emergencies and changes in resident condition. Qualifications Graduate of an accredited Licensed Practical Nursing (LPN) program. Current Licensed Practical Nurse (LPN) license in good standing with the Commonwealth of Pennsylvania. Previous long-term care, rehabilitation, or skilled nursing experience preferred, but not required. Current CPR certification preferred. Strong clinical, communication, organizational, and documentation skills. Ability to work collaboratively in a fast-paced healthcare environment.
