Monitor residents and promptly report changes in condition. Assist with care planning and ongoing resident assessments. Collaborate with RNs, CNAs, physicians, and other healthcare professionals. Maintain accurate and timely clinical documentation. Promote resident safety, comfort, dignity, and quality of life.
