The ideal candidate will possess a strong background in patient care and demonstrate exceptional clinical skills. Our medical practice is seeking a Licensed Practical Nurse (LPN) to support patient care coordination in collaboration with our providers. This role requires adherence to clinical protocols, regulatory standards, and best practices to ensure optimal patient outcomes. Primary Responsibilities Collaborate with Medical Assistants (MAs) and providers to support daily patient care operations. Maintain thorough and accurate documentation within our electronic health record system. Assist in monitoring practice-level quality measures and compliance standards. Qualifications Active and unrestricted Licensed Practical Nurse (LPN) license in Pennsylvania. A few years of experience in primary care, chronic disease management, or a related clinical setting preferred. Familiarity with electronic health record systems. Strong clinical judgment, attention to detail, and organizational skills. Excellent written and verbal communication skills. Schedule & Compensation Part-time position with flexible scheduling. Compensation commensurate with experience. Potential for expanded responsibilities as the role grows.
