Job Description
Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality, patient-centered care. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions and compassionate communication. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations while working in a slow paced, collaborative environment. Duties Perform comprehensive patient assessments including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols while adhering to HIPAA and clinical confidentiality policies. Manage IV infusions, catheter care, tube feeding, and ventilator support for patients with complex needs. Assist with diagnostic evaluations, specimen collection/processing, and wound care procedures such as suturing and sterile technique. Provide specialized care for patients with Alzheimer's disease, dementia, or memory impairments in memory or hospice care settings. Support acute pain management, infectious disease care, and emergency responses including airway management and first aid. Collaborate with interdisciplinary teams on discharge planning, case management, and patient education to ensure optimal outcomes across medical-surgical, geriatrics, pediatrics, ICU (Intensive Care Unit), trauma centers (Level I & II), and behavioral health environments. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with active certification. Proven experience working with diverse populations including geriatric patients, individuals with developmental disabilities, or those requiring specialized memory or dementia care. Demonstrated proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks; experience with
