Job Description

Job SummaryHamilton Health Center is seeking an LPN Care Coordinator to support patient-centered care at our Newport Family Care location.

This role promotes collaboration among the patient-centered care team to ensure quality care, increase preventive care utilization, and reduce emergency room visits and hospital readmissions. The LPN Care Coordinator provides care coordination and case management for patients, including individuals with higher-risk or complex care needs, while helping patients and families navigate healthcare services and treatment plans.

The position works collaboratively with providers, Medical Assistants, Patient Navigators, Pharmacy, and other members of the care team to support continuity of care and positive patient outcomes.
