Job Description

Community Based LPN Full Time — Monday through Friday (8:00 am - 5:00 pm; occasional evenings/weekends for community events) The Community Based LPN delivers direct patient care in the home and community, supporting Keystone Health providers during telehealth visits by performing prescribed treatments and facilitating access to care for patients facing medical, social, or transportation barriers. Working as part of an interdisciplinary team—including medical providers, RN Care Managers, and Case Managers—the LPN provides short‑term care coordination, connects patients with community resources, and addresses social drivers of health. The role also includes offering culturally appropriate education, social support, and informal counseling to help patients improve their health and self‑management. The LPN participates in community outreach and engagement activities to increase awareness of available services, with a focus on elderly and vulnerable populations. Education & Experience Graduate of an accredited Practical Nursing Program. Licensed Practical Nurse with at least two years of recent experience in an ambulatory care or physician office setting. Experience in community‑based roles (health educator, community health worker, etc.) is a plus.
