Provide individualized nursing care aligned with established resident care plans. Monitor resident health status, record vital signs, and promptly report clinical changes to supervising staff or physicians. Perform skilled nursing procedures, including wound care, catheter care, tube feedings, and blood glucose checks. Supervise, guide, and delegate tasks to CNAs and caregiving staff during shifts. Communicate clearly and compassionately with residents, family members, and interdisciplinary healthcare teams. Maintain accurate, timely clinical charting and documentation in electronic health record systems. Uphold infection control standards, resident rights, privacy regulations, and safety protocols at all times. Requirements Active, unencumbered Licensed Practical Nurse (LPN) license in Pennsylvania. Minimum of 6 months of clinical experience in long-term care, assisted living, or skilled nursing environments. Current Basic Life Support (BLS) / CPR certification. Experience working with senior care or memory care populations preferred. Ability to lift at least 50 lbs and assist residents with physical mobility needs. Strong clinical judgment, time management, and medication administration skills. Effective communication skills in English (reading, writing, and verbal). Must be at least 18 years of age.
