Job Description

Position Overview:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our growing team! In this role, you'll play a vital part in delivering skilled, patient-centered care in a supportive home health environment.

Key Responsibilities:

Provide direct nursing care under the supervision of an RN. Administer medications and treatments as prescribed. Monitor patient conditions and report changes to the care team. Document care accurately and timely. Communicate effectively with patients, families, and team members.

Qualifications:

Active LPN license in Pennsylvania . Prior home health or pediatric experience is a plus, but not required. Strong interpersonal and organizational skills. Valid driver's license and reliable transportation. CPR certification.
