We are seeking a dedicated and compassionate Evening Shift LPN to provide outstanding care to our patients and residents in Sunbury, PA. About the
Role:
As an Evening Shift LPN, you will play a vital role in delivering exceptional patient care and support to our residents.
Your primary responsibilities will include:
Providing direct patient care, including administering medications, wound care, and other treatments as needed Conducting vital sign checks, monitoring patient conditions, and reporting any changes or concerns to the healthcare team Assisting with mobility and ambulation, transferring patients, and providing support with daily living activities Maintaining accurate and detailed records of patient care and progress Collaborating with the interdisciplinary team to develop and implement individualized care plans Ensuring a clean and safe environment for patients and staff Participating in quality improvement initiatives and implementing changes as necessary
Requirements:
Valid LPN license in the state of Pennsylvania Minimum 1-year experience as an LPN in a long-term care or rehabilitation setting Strong communication and interpersonal skills Ability to work effectively in a fast-paced environment and prioritize tasks accordingly Commitment to delivering exceptional patient care and support Must be available to work evening shifts, rotating weekends and holidays
