Position Summary The LPN Nurse Assessment Coordinator will assist with coordinating resident assessments, maintaining accurate documentation, and supporting regulatory compliance while receiving hands-on training from an experienced RNAC.

Responsibilities:

Assist with the coordination and completion of resident assessments. Learn and support the MDS/RAI process under the guidance of an experienced RNAC. Maintain accurate and timely documentation. Collaborate with the interdisciplinary team to develop and update resident care plans. Monitor assessment schedules and assist with regulatory compliance. Participate in quality improvement initiatives and resident care planning.

Qualifications:

Current LPN license in good standing. Experience in longterm care or skilled nursing preferred. Knowledge of

MDS, RAI

process, and Medicare/Medicaid regulations is preferred. Strong organizational, communication, and computer skills. Ability to work independently while collaborating effectively with a multidisciplinary team.
