Qualifications:
Graduate of a Licensed Practical Nursing Program approved by the State Board of Nursing Minimum of 2 years of LPN experience, preferably with a background in home health care Active license and certifications in
Pennsylvania:
Licensed Practical Nurse (LPN) Basic Life Support (BLS) PA Act 31, PA Act 33, PA Act 34, PA Act 73
Responsibilities:
Deliver home health nursing care, ensuring patient safety and well-being Collaborate with interdisciplinary teams to implement individualized care plans Accurately document patient status, interventions, and responses Support patient and family education regarding ongoing care needs Adhere to all regulations and standards in clinical practice
