Both day shifts and overnight shifts! Part time and Full time options available. 8 and 12 hour shifts available. Trach and Vent experience required. Job Summary for LPN We are seeking a dedicated and Practical Nurse for Home Care (LPN) to join our professional care team. The ideal candidate will possess a strong clinical background, excellent patient assessment skills, and a passion for delivering exceptional home healthcare services. As an LPN in this role, you will coordinate and provide comprehensive nursing care to patients in their homes, ensuring safety, comfort, and optimal health outcomes through expert clinical judgment and compassionate support. Responsibilities of LPN Conduct thorough patient assessments to develop individualized care plans aligned with medical needs and patient preferences. Administer medications, following aseptic techniques and safety protocols. Monitor vital signs, perform physical examinations, and evaluate patient progress regularly. Provide specialized care such as wound suturing, ostomy care, catheterization, and ventilator management as required. Educate patients and their families on disease management, medication administration, and health maintenance strategies. Coordinate with physicians and other healthcare professionals to ensure continuity of care and appropriate interventions. Document all patient interactions accurately within electronic health record (EHR) systems while maintaining compliance with HIPAA regulations. Qualifications for LPN Valid Practical Nurse (LPN) license in the appropriate state or jurisdiction. Proven experience in home healthcare or related clinical settings such as critical care, hospital medicine, or long-term care environments. Experience with medical documentation standards. Strong clinical skills including IV insertion, medication administration, sterile technique, and infection control practices. Trach and Vent experience required (this assignment has a patient with trach/vent specifically). Excellent communication skills with the ability to educate patients effectively and collaborate within multidisciplinary teams.
