Full Job Description Core Duties & Responsibilities Position Summary:
The Enhanced Care Management (ECM) program is designed to support individuals with complex health and social needs. The LVN plays a critical role in clinical oversight for the department and all Lead Care Managers (LCMs) within it. This position is non-supervisory and functions as advisory support for the case management team. While the LVN provides clinical insight, they do not replace the member's Primary Care Provider, ensuring all medical advice is rooted in established care plans and instructions from the treating medical team. 1. Clinical Oversight & Care Plan Review Review and validate
Care Plans:
Acts as the primary clinical reviewer for member Care Plans created by Lead Care Managers (LCMs). Evaluates plans for clinical appropriateness, safety, and alignment with the member's current health statuses.
Sign-off Authority:
Provides the final clinical sign-off on Member Assessments and Care Plans, certifying that the proposed interventions meet the necessary clinical standards before implementation.
Clinical Consultation:
Analyzes gaps in care and provides specific, actionable feedback to LCMs regarding medical necessities. Suggests modifications to goals and interventions to ensure better health outcomes. Coordinates with department supervisor to ensure department is providing clinically appropriate support. 2.
Direct Member Engagement & Monitoring Monthly Clinical Contact:
Maintains a mandatory monthly touchpoint (telephonic or in-person) with the assigned member caseload to monitor health status, identify emerging clinical risks, and ensure continuity of care.
Medication Reconciliation:
Conducts thorough medication reconciliations (MedRecs) to ensure accuracy between the member's actual usage and provider prescriptions. Identifies potential interactions or adherence issues and escalates to the Primary Care Provider (PCP) when necessary.
Targeted Field Visits:
Conducts in-person clinical visits for high-acuity members or those with complex needs. These visits are coordinated via triage and LCM suggestions to assess the member's immediate physical environment and stability. 3. Interdisciplinary Team (IDT)
Collaboration Complex Case Consultation:
serves as the clinical subject matter expert during Interdisciplinary Team (IDT) meetings for complex cases. Translates medical jargon for non-clinical team members and advises on the medical management of chronic conditions.
Clinical Charting:
Documents high-quality, clinically accurate notes regarding IDT discussions, member interactions, and observations in the Electronic Health Record (EHR), ensuring audit-readiness and compliance. 4.
Health Education & Scope of Practice Adherence Patient Education & Coaching:
Provides general health education and coaching focused on chronic disease self-management. Strictly reinforces the treatment plans, instructions, and advice provided by the member's PCP and specialists.
Scope of Practice Compliance:
Operates strictly within the scope of an LVN. Does not diagnose conditions, prescribe treatments, or provide medical advice contrary to or independent of the member's treating physicians. Focuses on compliance, education, and care coordination. Scope and Limitations This role focuses on clinical coordination, education, and oversight. Qualifications and Requirements Minimum Requirements:
Licensure:
Must hold a valid, unrestricted license as a Licensed Vocational Nurse (LVN) in the State of California.
Experience:
Minimum of 2-3 years of clinical nursing experience required.
Certifications:
Current Basic Life Support (BLS) certification required.
Travel:
Must possess a valid driver's license, reliable transportation, and auto insurance.
Preferred Qualifications:
Specialized Knowledge:
Previous experience working with Medi-Cal populations or within an Enhanced Care Management (ECM) / CalAIM framework is highly preferred.
Complex Case Experience:
Experience working with individuals with complex health and social needs, including chronic disease management and social determinants of health.
Care Management:
Proven experience in case management, specifically in reviewing and validating care plans for clinical appropriateness and safety.
Key Competencies:
Clinical Assessment:
Ability to review non-clinical assessments and provide clinical sign-off, ensuring interventions meet medical standards.
EHR Proficiency:
Strong proficiency in conducting medication reconciliations (MedRec) and identifying drug-drug interactions. Experience with Electronic Health Records (EHR) systems and ability to produce audit-ready clinical notes.
Communication:
Ability to "translate" complex medical jargon into clear, understandable language for non-clinical team members (LCMs) and patients.
Advisory Capability:
Comfortable working in a non-supervisory, advisory capacity to guide Lead Care Managers rather than directing them administratively.
Technical Capacity:
Able to work with common office and productivity software (Office 365, Google Workspace, Excel, etc).
Physical Demands & Work Environment:
Field Work:
Ability to navigate various physical environments during home and community visits, which may include private residences and care facilities.
Mobility:
Requires distinct periods of sitting, standing, and driving. Must be able to travel to conduct in-person field visits for high-acuity members as needed.
Communication:
Frequent use of telephone and video conferencing equipment to maintain monthly contacts and participate in IDT meetings.
Pay:
$30.00 per hour Expected hours: 40.0 per week
