is looking for a compassionate and skilled Licensed Practical Nurse (LPN) to join our growing Home Health team serving Pasco County. Candidate will initiall work on a PRN basis. Responsibilities:

Provide skilled nursing care to home health patients throughout Pasco County in accordance with physician-approved care plans Perform clinical assessments, wound care, medication administration, and IV therapy as ordered Monitor and document patient progress, vital signs, and changes in condition Educate patients and family members on disease management, medications, and home care techniques Collaborate with RNs, therapists, and the interdisciplinary care team to ensure continuity of care Maintain accurate and timely clinical documentation in compliance with state and federal home health regulations Communicate patient status updates and concerns to supervising RN or physician promptly

Requirements:

Active Florida LPN license in good standing Familiarity with WellSk Previous home health, long-term care, or clinical nursing experience preferred Strong knowledge of wound care, medication management, and patient assessment Reliable transportation and valid driver's license for travel throughout Pasco County Excellent communication, time management, and organizational skills Ability to work independently with minimal supervision in a home care setting CPR/BLS certification required
