Job Description

Duties Job Summary We are seeking a Licensed Practical Nurse to fill our Client Coordinator Intake Specialist position. The LPN will assist with R.N. with coordinating all aspects of the client's in-home care, as indicated by Agency policies and State/Federal regulations. This includes admissions, in-home assessments, eligibility determinations and development of care plans. Duties Assess the home environment and determines needs of clients. Records the health needs of the clients. Initiates the care plans and all associated paperwork in accordance with the company's policies. Evaluate the effects of the care given and regularly re-evaluate the client's service needs. Evaluate and revise the care plan as needed. Makes referrals to other agencies or services as needed. Coordinates the care of all assigned clients. Conduct regularly required service reviews per company and state requirements. Instruct caregivers in the proper use of practices, procedures and equipment. Participate in weekly and monthly training programs and staffing meetings. Serve as a Liaison between clients and other healthcare professionals. Monitor clients and instruct clients on appropriate home care services. Counsel the client and family members on meeting related homecare needs. Act as the coordinator of the Health Care Team. Provides reports to the client's Service Waiver Care Manager and Care Coordinator when the client's condition changes or there are deviations from the Care Plan. Prepare clinical and progress notes on relevant home care services that the company provides. Maintain a clinical record for each client receiving care. Submit proper documentation per company policies. Complies with Agency Compliance Plan. Complies with Agency Infection Control Policies. Complies with Agency HIPPA Policies. Complies with all federal and state regulations and standards. Works weekends, holidays and overtime when requested. Projects a professional attitude towards self, clients, family memebers and staff members. Assume other duties as requested by the Agency Manager/Administrator. Qualifications Graduate of an accredited school of professional school/program of nursing and is currently licensed as an LPN through the Ohio State Board of Nursing. Home healthcare experience highly recommended. Case Management experience highly recommended. Has a minimum of one year of nursing experience in a LTC setting. Possess and maintain good shysical and mental health. Must have a current driver's license and insurance. Excellent writing and charting skills. Can pass a background check.
Pay:
$31.00 - $33.00 per hour
