The position follows an RN/LPN care model, with the RN serving as the Case Manager and the LPN providing care in accordance with the Case Manager's established plan of care and Physician orders.

Responsibilities:

Provide skilled nursing care to patients in their homes. Collaborate with the RN Case Manager and other members of the care team to ensure continuity and quality of care. Follow individualized plans of care and communicate changes in patient condition appropriately. Work collaboratively with a multidisciplinary team, including Physical Therapy (PT), Occupational Therapy (OT), Speech-Language Pathology (SLP), and Medical Social Work (MSW) . Promote patient independence, safety, and quality of life while allowing patients to remain in the comfort of their own homes. Maintain accurate and timely clinical documentation. Service Area Northern Rhode Island, including: Woonsocket Smithfield North Smithfield Surrounding Northern RI communities as needed

Requirements:

Current license to practice as an LPN in RI. Valid CPR license. Valid driver's license and proof of insurance coverage. Ability and willingness to work in a team environment. Ability to travel between patient assignments within a 25-mile radius. Experience in Home Care a plus but not required.
