you'll contribute to our success in the following ways: Follow each patient's individualized care plan and physician orders. Monitor patient progress and report changes or safety concerns to the care team. Help patients and caregivers learn self-care techniques to support their health. Work with the RN Case Manager to update care plans as patient needs change. Administer and document medications according to agency guidelines. Communicate with physicians, patients, families, and the care team to ensure coordinated care. Help prevent avoidable hospitalizations and emergency room visits through proactive patient care. Deliver compassionate, high-quality care that supports positive patient outcomes. What is Required? Current State License as a Licensed Vocational / Practical Nurse Graduate of an accredited Certificate, Diploma, or Associate School of Nursing 1 year of experience in a clinical care setting Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Experience in a hospice or home health environment is preferred What's in it for you?
