Job Description

Overview Join our dedicated healthcare team as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) specializing in Long Term Care. In this vital role, you will provide compassionate, high-quality nursing care to residents in a long-term care setting, ensuring their comfort, safety, and well-being. You will be instrumental in delivering personalized care plans, managing complex medical needs, and fostering a supportive environment for residents with diverse health conditions. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring ongoing medical support and assistance. Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to inform individualized care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management using EMR/EHR systems such as Epic or Cerner. Monitor residents' conditions closely, utilizing telemetry and other advanced monitoring tools to detect changes in health status promptly. Provide specialized care for residents with memory care needs, dementia, or developmental disabilities, ensuring a safe and supportive environment. Perform wound care procedures such as suturing and ostomy care while maintaining strict aseptic techniques to prevent infections. Assist with activities of daily living (ADLs), including tube feeding, catheter care, and mobility assistance using equipment like Hoyer lifts or wheelchairs. Collaborate with interdisciplinary teams on discharge planning, case management, and compliance with regulatory standards such as NCQA standards and HIPAA regulations. Respond effectively to urgent situations including acute pain management, trauma incidents at Level I or Level II trauma centers experience preferred, and emergency response procedures like airway management or neonatal resuscitation when necessary. Experience Proven experience working in long-term care facilities such as nursing homes or assisted living communities. Critical care experience in ICU or emergency room settings is highly valued for managing complex cases. Familiarity with EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation and medical records management. Knowledge of
ICD-10/ICD-9
coding for diagnostic evaluation and billing purposes. Hands-on experience with IV insertion, phlebotomy, laboratory specimen collection/processing, and infection control protocols. Background working with diverse populations including seniors with geriatrics needs, behavioral health challenges, or individuals with disabilities requiring specialized attention like dementia or traumatic brain injury care. Experience in pain management techniques and medication administration tailored to acute or chronic conditions across various medical settings such as hospital medicine or outpatient clinics. Demonstrated ability to perform physical examinations, vital signs assessment, and patient education within a fast-paced environment that emphasizes patient-centered care.
