Duties Provide strategic oversight of home care operations, ensuring high-quality patient care aligned with agency goals and regulatory standards Supervise and support Home Health Aides/CNAs fostering a collaborative team environment Develop, review, and update individualized care plans in accordance with residents' needs, including those requiring memory care or hospice services Ensure compliance with all federal, state, and local regulations. Oversee documentation review of medical records, clinical notes, and care documentation. Manage staffing schedules, shift assignments, and resource allocation to optimize patient care delivery and staff workload balance Lead quality assurance initiatives by monitoring patient outcomes, conducting audits of medical records, and implementing process improvements Requirements Valid RN or LPN license with active licensure in the state of practice Proven management or nurse management experience within home health agencies or long-term care facilities Strong knowledge of resident's rights, nursing home regulations, long-term care regulations, and Medicare requirements Familiarity with EMR/EHR systems for accurate recordkeeping and reporting Excellent understanding of medical terminology, physiology, anatomy, and memory care principles Demonstrated leadership skills in supervising teams while maintaining compliance with HIPAA privacy standards Ability to manage multiple priorities effectively across shift management and administrative responsibilities Available on call for scheduling and client needs Join us in leading compassionate home health services that prioritize patient dignity, safety, and well-being.
