Job Overview We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality nursing care to patients across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through comprehensive assessments, medication administration, and personalized care plans. The ideal candidate will demonstrate strong clinical skills, excellent communication abilities, and a commitment to delivering respectful, patient-centered care. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to inform individualized care plans. Administer medications and injections accurately following prescribed protocols while monitoring for adverse reactions. Perform specialized procedures such as IV infusions, catheterizations, tube feedings, and wound care with precision and aseptic technique. Document patient information meticulously in electronic health records (EHR/EHR systems), ensuring compliance with HIPAA and confidentiality policies. Collaborate with interdisciplinary teams to coordinate care, discharge planning, and case management for diverse patient populations. Provide emergency response and triage support in acute care settings such as emergency rooms or trauma centers. Support patients with chronic conditions like Alzheimer's disease, dementia, or developmental disabilities through tailored interventions. Educate patients and families on medication management, health maintenance, and post-discharge instructions. Maintain a safe environment by adhering to infection control standards and safety protocols. Assist in diagnostic evaluations and prepare specimens for laboratory processing when required.
