Job Description

Overview Join our dedicated home care team as a Licensed Practical Nurse (LPN) and make a meaningful difference in the lives of patients requiring compassionate, expert nursing care in their own homes. In this vital role, you will provide personalized nursing services, promote health and safety, and support clients through their recovery and ongoing health management. Duties Perform comprehensive patient assessments including vital signs, physical examinations, and medication reconciliation to develop individualized care plans. Administer medications via injection, IV infusion, catheterization, and other methods following strict protocols to ensure safety and efficacy. Manage complex medical needs such as wound care, tube feeding, ventilator support, and pain management for patients with acute or chronic conditions. Utilize electronic health record (EHR) systems like Epic or Cerner to document patient interactions accurately while maintaining strict compliance with HIPAA and clinical confidentiality policies. Provide education to patients and caregivers on disease management, medication administration, mobility assistance, and safety procedures. Coordinate with multidisciplinary teams including physicians, therapists, and case managers to ensure seamless continuity of care. Respond promptly to triage calls or urgent situations by assessing patient needs and initiating appropriate interventions or referrals. Experience Valid LPN license with current state licensure required; additional certifications such as Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) preferred. Demonstrated experience working with individuals with disabilities, dementia care including Alzheimer's disease and memory care populations. Proven background in diverse healthcare environments such as hospitals (including ICU or trauma centers), nursing homes, skilled nursing facilities, hospice or home care settings. Familiarity with EMR/EHR systems like Epic, Athenahealth or eClinicalWorks; experience with ICD coding (ICD-9/10), CPT coding is advantageous. Critical care experience including acute pain management, infectious disease care, ventilator management or emergency medicine is highly valued. Knowledge of long-term care regulations, geriatrics principles, and best practices for patient assessment and documentation. Ability to communicate effectively with patients of all ages—including pediatrics—and collaborate within multidisciplinary teams to deliver holistic care.
