Job Description

Overview Join our dedicated home care team as a Home Care Licensed Practical Nurse (LPN), working Monday through Friday to provide compassionate, high-quality nursing care in patients' homes. This vital role offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring personalized health support. As an LPN, you will deliver skilled nursing services, promote patient comfort, and ensure adherence to all safety and health regulations within a home setting. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation to inform personalized care plans Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards Manage complex care needs such as wound care, catheterization, tube feeding, and ventilator management with precision and compassion Document all patient interactions accurately using Electronic Health Records (EHR) systems like Epic or Athenahealth while maintaining strict compliance with HIPAA and confidentiality policies Educate patients and their families on disease management, medication routines, and health coaching strategies to foster independence and wellness Coordinate with physicians, case managers, and other healthcare professionals for seamless discharge planning and ongoing care delivery Respond promptly to urgent patient needs through triage skills, including acute pain management and infectious disease care Skills Proven experience working with individuals with developmental disabilities, dementia (including Alzheimer's and memory care), or in long-term care environments such as skilled nursing facilities or assisted living communities Strong background in critical care settings such as ICU or Level I Trauma Centers; experience with emergency medicine or hospital medicine is highly valued Proficiency in medical documentation using EMR/EHR systems like Cerner or eClinicalWorks; familiarity with
ICD-9/10
coding is preferred Skilled in advanced nursing procedures including IV insertion, phlebotomy, sterile technique, catheter care, and first aid techniques Knowledge of physiology, anatomy, pharmacology, and medical terminology necessary for accurate patient assessment and treatment planning Ability to communicate effectively with patients of all ages—including pediatrics (PICU experience) and geriatrics—while maintaining clinical confidentiality policies Experience working with diverse populations such as those with disabilities or chronic conditions like dementia or hospice needs is a plus Demonstrated ability to work independently in home environments while adhering to long-term care regulations and safety protocols Embark on a rewarding career where your skills directly enhance the quality of life for those you serve.
