Job Description
Overview We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) with experience in Pediatrics to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including client's home, outpatient facilities and long-term care environments. As an LPN, you will play a vital role in supporting patients' health and well-being through direct care, case management, and effective communication with healthcare teams. Duties Provide direct patient care, including administering medications, feeding tubes, and treatments as prescribed. Collaborate with healthcare professionals to develop and implement individualized care plans. Conduct patient assessments and monitor vital signs to track health status. Maintain accurate patient records and documentation in compliance with healthcare regulations. Support case management efforts by coordinating with multidisciplinary teams to address patient needs. Engage in hospital medicine practices, ensuring adherence to established protocols and standards of care. Foster a caring environment that promotes patient dignity and respect. Current LPN licensure in the state of South Carolina Experience in outpatient settings or long-term care preferred. Strong understanding of discharge planning processes and hospital medicine practices. Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare teams. Ability to manage multiple tasks efficiently while maintaining attention to detail. Must have a minimum of 2 years of experience with the use of tracheostomy, gastric or jejunostomy tubes, and indwelling catheters.
