Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across diverse settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. Your expertise will help ensure patients receive personalized treatment, comfort, and support while maintaining compliance with healthcare regulations. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals with varying health needs, from geriatrics and memory care to acute and emergency situations. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications, injections, IV infusions, and other treatments following established protocols and physician orders. Manage complex care needs such as dementia care, Alzheimer's support, hospice services, and pain management for diverse patient populations. Assist with diagnostic evaluations like specimen collection, laboratory testing, and medical imaging procedures. Operate medical equipment including ventilators, catheters, and wound care supplies while adhering to aseptic techniques and infection control policies. Document all patient interactions accurately using Electronic Health Records (EHR) systems such as Epic or Cerner; ensure compliance with HIPAA regulations. Collaborate closely with physicians, case managers, caregivers, and families to coordinate discharge planning and long-term care strategies. Experience Proven experience working in various healthcare settings such as hospitals nursing homes, skilled nursing facilities, or outpatient clinics. Demonstrated proficiency in critical care areas like emergency medicine, ICU nursing, or Level I/II trauma centers is highly desirable. Experience working with individuals with disabilities or developmental challenges; expertise in dementia or memory care is a plus. Strong understanding of long-term care regulations and best practices for senior care and hospice services. Ability to perform patient assessments in diverse environments including home & community care settings or travel nursing assignments. $32.00 - $35.00 per hour Expected hours: 20.0 - 40.0 per week
