Licensed Practical Nurse (LPN) - Long Term Care Job Description The Licensed Practical Nurse (LPN) provides compassionate, direct nursing care under the supervision of a Registered Nurse (RN). The LPN administers medications and treatments, monitors patient conditions, assists with care planning, and ensures compliance with infection control and facility standards. Responsibilities Provide direct nursing care and assist with personal hygiene and daily living needs. Administer medications and treatments, including respiratory therapy as prescribed. Monitor patients and report changes in condition to the RN. Maintain accurate and timely patient documentation. Follow infection control and safety policies. Respond appropriately to emergencies and changes in patient condition. Provide patient education and discharge instructions. Assist with the orientation and training of new staff members and students. Maintain clinical competency through continuing education. Demonstrate professionalism, patience, compassion, and effective communication. Requirements Graduate of an approved LPN program. Current LPN license. Current CPR and ACLS certifications. Basic computer skills and the ability to learn facility software. Ability to assess patient needs and provide safe, effective nursing care. Long Term Care (LTC) experience preferred.
