Job Description
CareRite Centers seeks a compassionate Licensed Practical Nurse to provide resident-centered care in our skilled nursing and rehabilitation facility. The LPN delivers bedside nursing, administers medications and treatments, monitors resident conditions, and documents in EMRs. You'll collaborate with an interdisciplinary team to implement individualized care plans, support rehab and long-term care goals, and educate residents and families. Responsibilities Deliver direct bedside nursing care to residents in a skilled nursing and rehab setting Administer medications and treatments per provider orders and facility protocols Monitor, document, and report changes in resident condition using EMR systems Collaborate with RNs, therapists, and physicians on individualized care plans Provide wound care, respiratory support, and rehab-focused interventions as ordered Educate residents and families on care plans, medications, and health management Support a compassionate, person-centered, and dignified environment for residents Adhere to infection control, safety, and regulatory standards at all times Supervise and support CNAs to ensure high-quality daily care Participate in interdisciplinary team meetings and ongoing professional development Required Skills Medication administration Wound care Tracheostomy and respiratory care IV therapy and injections Electronic medical records (EMR) documentation Vital signs and patient assessment Care planning and care coordination Infection control and prevention Rehabilitation and restorative nursing techniques Patient and family education
