Job Description
Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and home health environments. Your expertise in patient assessment, medication administration, and clinical procedures will ensure our patients receive safe, effective, and personalized care. Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures. Manage acute pain and provide specialized dementia or Alzheimer's care tailored to individual needs. Support patients with complex conditions such as ventilator management, tube feeding, or wound care while adhering to infection control policies. Document all patient interactions thoroughly within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations. Collaborate with interdisciplinary teams on discharge planning, case management, and client communication to optimize patient outcomes. Experience Proven experience working as a Licensed Practical Nurse in diverse healthcare settings such as hospitals (including ICU or Level I Trauma Centers), nursing homes, or outpatient clinics. Demonstrated proficiency in EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation. Critical care experience including ICU or emergency room environments is highly desirable. Familiarity with medical coding systems such as ICD-9 and ICD-10 for accurate documentation and billing purposes. Experience working with individuals with disabilities or those requiring specialized memory care or hospice services is preferred. Knowledge of long-term care regulations and geriatrics to deliver safe and compliant senior care services. Additional skills in phlebotomy, sterile processing, first aid, and advanced life support (CPR/BLS) are advantageous for this role.
