Job Description:

We are currently looking to hire a Full-Time LPN for the Night Shift shift: 7PM-7AM. This person works under the supervision of the Director of Nursing, providing nursing care to residents, and performing assigned nursing procedures for our residents' comfort and well-being. In conjunction with the D.O.N., the LPN assumes responsibilities for the facilitation and coordination of all activities relevant to the healthcare of the residents during the day shift. The LPN provides direct nursing care to residents who require professional, skilled attention, in a compassionate, professional, ethical, and knowledgeable manner.

The LPN also:

• Administers medications and treatments in accordance with established policies and procedures. Is familiar with indication for use, action and major side effects of medications administered.
• Collaborates in the development and modification of the comprehensive nursing plan of care for all patients. Implements aspects of the nursing care plans, reflecting physician's orders and diagnosis.
• Takes direct, appropriate action in urgent and emergent situations.
• Makes rounds on residents at least three times each shift, collecting data and collaborating in assessing for changes in condition and response to care and treatment.
• Monitors the status of residents with acute problems
• Communications and Interpersonal Care
• Serves as an advocate for the Residents, by communicating appropriately with the Director of Nursing, entire community, co-workers, other employees and visitors.
• Maintains a therapeutic relationship with residents.
• Documents appropriate and necessary information in the medical record.
• Exchanges pertinent information during inter-shift report.
• Maintains a cooperative relationship within health care team by communicating information, responding to requests, building rapport and participating in team problem-solving methods.
• Supports MQA's policies and procedures and the Nursing Department's standards of care.
• Observes and preserves resident's rights.
• Channels administrative problems appropriately to the Director of Nursing.
• Demonstrates knowledge of fire and safety plans, and assists the staff in charge with evacuation procedures. Also demonstrates knowledge of universal body substance isolation practices.
• Actively participates in staff development programs and unit meetings.
• Seeks supervision and guidance as needed.

Applicable knowledge and skills:

• Comprehensive knowledge of nursing care, medications, gerontology, etc.
• Excellent interpersonal, oral and written communication skills.
• Ability to lift, stand, push, pull, bend, stretch and move residents.
• Must understand English well enough to deliver required care to the residents, communicate clearly with residents, employees, visitors, etc.

Education and Experience:

• Current license to practice as an LPN/RN in Tennessee state ; CPR certification.
• Graduation from an approved Licensed Practical Nursing program.
• At least 1 year experience working in assisted living or nursing home environment.

Pay Determined by license type & combined experience.
