Job Description
Clinical Responsibilities Health Assessments:
Conduct ongoing physical, cognitive, and psychosocial evaluations to identify changes in resident conditions.Care Planning:
Develop, update, and implement individualized care plans based on assessments and resident goals.Medication Management:
Administer and supervise medication delivery, monitor side effects, and ensure regulatory compliance. righteo.com.auChronic Condition Management:
Monitor and manage chronic illnesses such as diabetes, heart disease, and dementia.Emergency Response:
Provide immediate clinical intervention and decision‑making during medical emergencies.Leadership and Coordination Duties Staff Supervision:
Train, mentor, and supervise care aides.Care Coordination:
Communicate with physicians and families to ensure continuity of care.Documentation:
Maintain accurate clinical records, incident reports, and regulatory documentation.Quality and Compliance:
Uphold facility policies, infection control protocols, and regulatory standards.Resident Support and Advocacy Emotional and Social Support:
Promote resident independence, wellbeing, and dignity through person‑centered care.Family Communication:
Act as a liaison to ensure families understand care plans, health changes, and treatment decisions.Risk Identification:
Monitor for fall risks, wandering, behavioral changes, and other safety concerns.Shift:
