The LPN/RN provides support and leadership to ensure resident quality of life by assisting with daily living activities and administering medications to residents per physicians' orders, quality assurance measures, state, federal and corporate regulations, and guidelines. Present a professional image & exhibit a manner that is cordial and respectful Communicate and maintain positive relationships with employees, residents, and family members Use integrity, good judgement, and problem-solving skills in making decisions Compile and maintain accurate reporting as required/requested Promote a safe and clean working environment Answer call lights and requests in a professional timely manner Maintain confidentiality of all resident information and ensure resident privacy Report observed changes in resident's condition or behavior promptly to supervisor Record and communicate appropriately in the resident's chart and to supervisor Order and receive medications Accurately document medication administration Provide quality resident care following their individualized care plans Assist residents in making doctor appointments and travel arrangements Assist with maintaining the care team schedule Lead resident care team by setting a professional example Assist in providing proper training to provide the highest quality of care to the residents Lead change of shift meetings, encourage active participation and teamwork within and amongst all shifts Must be willing to fill open shifts (1st, 2nd, and 3rd shifts) Make resident safety and satisfaction a priority Report to work on-time and ready for the workday Attend/complete required in-services and other mandatory trainings and meetings Other duties as assigned
Requirements:
LPN or RN 1-3 years of Healthcare, Assisted Living, Memory Care experience preferred
