Role:

Licensed Practical Nurse (LPN)

Responsibilities:

Provide resident-centered care, administer medications and treatments, monitor resident conditions, and document in EMRs.

Collaboration:

Work with an interdisciplinary team to develop and implement individualized care plans, support rehabilitation and long-term care goals, and educate residents and families.

Qualifications & Skills:

Compassionate, skilled in bedside nursing, medication administration, and documentation, with strong communication and teamwork abilities.

Key Duties:

Promote dignity, safety, and comfort for residents while contributing to a respectful, family-like environment and supporting residents' well-being and recovery.
