Job Description

Trillium Wellness Center is a 16-bed adult residential mental health facility located in Seymour, Tennessee. We provide compassionate, individualized care in a calm and supportive environment. We are seeking a dependable and compassionate Licensed Practical Nurse (LPN) to join our multidisciplinary treatment team. This position focuses on medication management, client monitoring, nursing intakes, vital signs, safety rounds, documentation, and coordination with our psychiatric and clinical providers. Responsibilities Administer scheduled and PRN medications according to provider orders Accurately document medication administration in the electronic medication administration record Monitor clients for medication effectiveness, side effects, adverse reactions, and changes in condition Complete medication reconciliation during admissions and discharges Maintain accurate medication and controlled-substance counts Coordinate medication refills with pharmacies and prescribing providers Complete nursing intakes for newly admitted clients Review medical history, current medications, allergies, dietary needs, and relevant health concerns Obtain and document vital signs, weights, blood glucose readings, and other ordered measurements Conduct scheduled nursing and safety rounds throughout the facility Monitor clients' physical health, mental status, behavior, sleep, and overall functioning Promptly communicate changes in condition or safety concerns to the appropriate provider Assist with crisis response, de-escalation, first aid, and emergency procedures Coordinate outside medical appointments, laboratory services, pharmacy deliveries, and follow-up care Provide client education regarding medications, general wellness, sleep hygiene, nutrition, and healthy routines Complete accurate and timely documentation in the electronic medical record Provide clear shift-to-shift nursing reports Participate in treatment-team meetings and communicate with clinical and support staff Maintain an organized medication room and adequate nursing supplies Follow all HIPAA, infection-control, safety, medication-management, and facility policies Maintain appropriate professional boundaries and treat every client with dignity and respect Perform additional nursing duties necessary for the safe operation of the residential program Qualifications Current and unrestricted Tennessee LPN license Current CPR/BLS certification or ability to obtain before beginning employment Previous behavioral health, psychiatric, residential treatment, or substance-use treatment experience is preferred Medication-administration experience required Experience with Kipu or another electronic medical record system is a plus Strong observation, documentation, organization, and communication skills Ability to remain calm and use sound judgment during medical or behavioral concerns Comfortable working with adults experiencing anxiety, depression, trauma, mood disorders, and other mental health conditions Ability to work independently while collaborating with a multidisciplinary team Must successfully complete all required background checks and pre-employment requirements Trillium Wellness Center is an equal-opportunity employer committed to maintaining an inclusive and respectful workplace.
