Responsibilities Perform comprehensive patient assessments, including vital signs, and physical examinations. Administer medications and injections accurately following physician orders while adhering to safety protocols and HIPAA regulations. Assist with complex medical procedures such as sterile dressing changes using aseptic techniques. Utilize EMR (Electronic Medical Records) system for accurate documentation of patient care activities. Support patients with neurological conditions such as Alzheimer's disease or dementia through memory care strategies and behavioral health support. Provide specialized care for patients with infectious diseases, acute pain management needs. Educate patients and their families on health coaching topics including medication management, nutrition, and chronic disease management. Maintain compliance with all healthcare regulations including HIPAA standards and NCQA (National Committee for Quality Assurance) guidelines. Ensure accurate medical documentation and detailed nursing notes. Participate in infection control practices and sterile processing procedures to uphold safety standards across all clinical activities. Experience Candidates should possess a valid LVN license with active certification to practice in their respective state. Prior experience is highly valued in the following areas: Long-term care facilities including nursing homes, assisted living communities, or sub-acute care units with a focus on geriatrics and dementia/dementia-related conditions like Alzheimer's disease. Experience working with diverse populations such as children (including Toddler or Neonatal care), adults in outpatient clinics, or home & community-based care environments. Proficiency in advanced clinical procedures such as phlebotomy, venipuncture, and specimen collection/processing. Additional desirable skills include experience administering injections; ability to perform physical examinations; understanding of physiology and anatomy; expertise in pain management strategies; and strong case management capabilities.
