We at Chaim Gitelis DO PC are a growing preventative healthcare company that partners with long-term care facilities to provide innovative healthcare solutions through Remote Patient Monitoring (RPM) and Chronic Care Management (CCM). We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team full-time and support residents within partner facilities.
Position Overview:
The LPN will play a vital role in bridging patient care with technology by monitoring vital signs, facilitating telehealth visits, and being a point of communication with residents enrolled in our RPM and CCM programs.
Responsibilities:
Take and record vital signs for residents enrolled in the RPM program using company-provided equipment. Facilitate telehealth visits and calls between residents and Nurse Practitioners (NPs). Conduct RPM and CCM tasks for assigned patients, either remotely or in a quiet designated area of the facility. Maintain accurate documentation of all patient interactions and vitals. Collaborate with NPs, facility staff, and our health care team to ensure seamless quality care.
Qualifications:
Active LPN license in the state of TX. Strong clinical and documentation skills. Comfortable with telehealth platforms and medical technology (training provided). Excellent communication and interpersonal skills. Reliable, organized, and able to work independently within a facility setting.
Schedule:
Full-time position Standard daytime hours with some flexibility based on facility needs
Compensation & Benefits:
