Community Healthcare of Texas is currently recruiting for an Inpatient
Assist the RN in the admission of a patient to Hospice House, including immediate crisis management and orientation. Assist RN in the discharge of patients and coordination of transfer, including instructing patients/caregivers in follow-up care. Assist the RN in the management of multiple patients and their support persons simultaneously in crisis with acute symptoms, including pharmacology.

LVN.POSITION SUMMARY
Assist the RN in the overall plan of care for the patient.ESSENTIAL FUNCTIONS
Assess the patient's physical, psycho-social, and spiritual condition; contribute to the plan of care in collaboration with an RN and provide palliative nursing care. Provide emotional support for patients and families/caregivers while instructing in end-of-life care/issues. Assists RN in case management of total plan of care of patient/support persons by coordinating with team members, communicating with IDG, and supervising CNAs. Document nursing care and significant interactions with patient support persons and team members. Participate in follow-up with bereaved families with various types of contact.Assist the RN in the admission of a patient to Hospice House, including immediate crisis management and orientation.
