Duties Provide direct patient care by performing assessments, administering medications, and monitoring vital signs in accordance with physician orders and established protocols. Assist with complex procedures such as IV infusions, catheterizations, wound care, and tube feedings while maintaining aseptic techniques. Document all patient interactions accurately using Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA and confidentiality policies. Collaborate with physicians, therapists, and other healthcare professionals to develop and update individualized care plans tailored to each patient's needs. Conduct triage assessments for incoming patients or residents, prioritizing urgent cases such as acute pain or infectious disease symptoms for immediate attention. Support long-term care residents by providing dementia care, memory support, and specialized assistance for individuals with Alzheimer's or other cognitive impairments. Participate in infection control practices, emergency response procedures including CPR and first aid, and maintain clinical equipment such as ventilators or oxygen therapy devices. Qualifications Valid LVN license issued by the state licensing board with current certification. Proven experience in critical care settings such as ICU or trauma centers is highly desirable. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth; experience with medical documentation is essential. Strong knowledge of medical terminology, anatomy, physiology, and nursing best practices across various specialties including geriatrics, pediatrics, emergency medicine, and hospice care. Experience working with diverse populations including individuals with developmental disabilities or those requiring specialized dementia or memory care. Ability to perform complex clinical tasks such as venipuncture (phlebotomy), injection administration (including vaccines), sterile processing techniques, and airway management. Excellent communication skills for caregiver interaction, patient assessment, family education, and interdisciplinary collaboration. Knowledge of long-term care regulations and ICD coding (ICD-9/10) standards for accurate documentation and billing purposes.
