In this vital role, you will provide compassionate, skilled nursing support across a variety of settings, including long-term care, hospitals, outpatient clinics, and community health environments. Your energetic approach and dedication will help ensure patients receive top-quality care tailored to their individual needs. This position offers a fulfilling opportunity to work with diverse populations, including seniors, individuals with disabilities, and those requiring acute or chronic care management. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following physician orders while adhering to HIPAA and safety protocols. Assist with medical procedures such as IV infusions, catheterization, wound suturing, and sterile dressing changes using aseptic techniques. Manage patient documentation efficiently within EMR (Electronic Medical Records) systems like PointClickCare. Conduct patient education on medication management, nutrition, disease processes like dementia or Alzheimer's care, and post-discharge instructions. Provide specialized care for patients with complex needs such as ventilator management, tube feeding, or pain control in acute care or hospice settings. Collaborate with interdisciplinary teams for discharge planning, case management, and coordination of home & community care services. Experience Valid LPN/LVN license in good standing with relevant state licensing boards. Prior experience working in medical-surgical units, emergency rooms (ER), ICU or Level I Trauma Centers is highly desirable. Knowledge of infectious disease protocols and infection control standards across various healthcare settings. Background working with diverse populations including pediatrics, geriatrics, individuals with developmental disabilities or behavioral health needs.
