Day's Pay Rate:

$1900-$2050/week

Responsibilities:

• Provide skilled nursing care to patients in their homes across assigned territories.
• Complete routine home health visits and meet the required 32 points per week .
• Assess patients, monitor their conditions, and provide appropriate nursing interventions.
• Develop and follow individualized patient care plans.
• Document patient care accurately using HomeCare HomeBase (HCHB) .
• Coordinate care with physicians, patients, families, and the interdisciplinary team.
• May be required to cover additional territories within approximately a 60-mile service radius .
• Follow all facility policies, including obtaining written approval for any overtime .

Requirements:

• Active LVN/LPN license in the applicable state.
• Recent home health experience (2024-2025) required.
• HomeCare HomeBase (HCHB) experience required ; candidates without HCHB experience may be considered at a reduced rate for the first 2 weeks if training is approved.
• Ability to complete routine home health visits and meet weekly productivity expectations.
• Reliable transportation and willingness to travel within the assigned service area.
• Strong clinical, communication, documentation, and time-management skills.
