(915) 587-5284 ________________________________________________________________________________________________ Responsiblites will include: Working under the supervision of a Registered Nurse to provide safe and compassionate nursing care as assigned. Providing care and assessing elder patients with chronic conditions, post-acute recovery, and patient disease care and education. Identify and report alterations in the client's response to nursing interventions to the Supervising Nurse. Document nursing care accurately, completely, and timely following the Agency's process, industry standards, nursing protocols, and state and federal regulations. Contribute to the plan of care by collaborating with the interdisciplinary team members, the client, and the client's family. May serve on the Interdisciplinary Team and/or the Quality Assessment Performance Improvement Committee. Maintain positive communication and cooperation with the clients, families, coworkers, and the community. Comply with all of the Agency's applicable policies. Work the assigned schedule and comply with the Agency's timekeeping policy. Other duties as assigned by the Administrator, or designee, when the needs of the Agency or its clients dictate. Job Qualifications/ Skills Must be licensed as an LVN by the State of Texas or a state with reciprocity through the

Nurse Licensure Compact Education/Training:

Graduate from an approved vocational nursing education program; maintain Continuing Nursing Education (CNE) requirements; current

CPR Experience:

One year of LVN experience, in home health preferred not required. Knowledge of nursing principles, procedures, and techniques; Knowledge of state and federal rules, regulations, and standards related to home health and to nursing per the Texas Board of Nursing. Skill in the use of general office equipment and the operation of computers. Detail oriented. Knowledge of how to develop constructive and cooperative working relationships with others; Skill in using reasoning and the Nursing Process to identity and solve issues related to the client's health Job type : Part-time, Per Diem Work location : In person/Patient's Home.
