Job Overview We are seeking a Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication administration techniques. Assist with complex procedures such as catheterization, tube feeding, sterile processing, and airway management. Provide specialized care for patients with conditions like dementia, Alzheimer's disease, or those requiring memory care or hospice services. Support acute care needs in settings such as Level I Trauma Centers or ICU environments by managing trauma protocols and emergency interventions. Conduct triage assessments to determine patient urgency levels in urgent care or emergency room settings; coordinate diagnostic evaluations and laboratory specimen collection/processing. Maintain meticulous medical documentation adhering to HIPAA regulations and clinical confidentiality policies while ensuring compliance with long-term care regulations. Skills Proven experience working with diverse populations including geriatric patients, individuals with developmental disabilities, children, and neonatal or pediatric patients. Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), and medical imaging procedures. Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth or similar platforms for documentation and patient records management. Skilled in infection control practices, aseptic technique, venipuncture (phlebotomy), catheter care, and basic life support (CPR). Ability to perform physical examinations and diagnostic evaluations accurately; experience with ventilator management and oxygen therapy preferred. Excellent caregiver communication skills to effectively collaborate with interdisciplinary teams and educate patients/families on health coaching and post-discharge care plans. Familiarity with acute pain management strategies, hospice care protocols, trauma response procedures (Level II/Level I Trauma Center experience), and working within hospital medicine or skilled nursing facility environments.
