Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed medical histories to inform individualized care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheterizations. Assist with complex procedures such as sterile dressing changes, tube feedings, airway management, and specimen collection/processing. Monitor patients' conditions closely, recognizing signs of acute pain, infectious disease symptoms, or neurological changes associated with dementia or Alzheimer's care. Document all patient interactions meticulously using Electronic Health Records (EHR) systems like Epic or Cerner to ensure compliance with HIPAA and long-term care regulations. Collaborate effectively with interdisciplinary teams—including physicians, case managers, and caregivers—to coordinate holistic patient care. Educate patients and their families on health management strategies, medication administration, and post-discharge care instructions to promote recovery and independence. Skills Proven experience working with diverse populations such as geriatric patients, individuals with developmental disabilities, or those requiring memory or dementia care. Strong knowledge of medical terminology, physiology, anatomy, and ICD coding (ICD-9/10) for accurate documentation and diagnostic evaluation. Proficiency in EMR/EHR systems like Epic, Cerner, Athenahealth or similar platforms for efficient medical records management. Skilled in performing advanced nursing procedures including IV insertion/infusion, phlebotomy, venipuncture, catheter care, and sterile techniques. Experience in acute care environments such as ICU or Level I Trauma Centers is highly desirable; familiarity with emergency medicine protocols is a plus. Ability to work effectively in fast-paced settings like emergency rooms or urgent care clinics while maintaining attention to detail. Excellent communication skills to collaborate with multidisciplinary teams and provide compassionate patient-centered care within HIPAA guidelines.
